Management Service
Case Management serves
youth who are transitioning to or from intensive services. Our case managers
work with the youth, family, referring agency, and providers to seek the least
restrictive and most appropriate environment. We draw on each youth's individual
strengths as well as assessment data to determine appropriate placement. Case
management services include assessment and evaluation, treatment plan monitoring
and development, linkage to community-based services, monitoring of services,
advocacy, resource development, and outcomes development.
 You
can earn a $100 bonus for referring a a qualified
therapeutic foster parent to First Home Care. Call the
